IMPORTANT NOTICE RE COVID-19: Please note any interaction with the general public poses an elevated risk of being exposed to COVID-19. We cannot guarantee that you will not be exposed while attending Carnegie Center classes and events.
WHAT WE’RE DOING TO KEEP YOU SAFE AND HEALTHY:
• We use air purifiers/scrubbers in our classrooms as well as fans to promote air circulation.
• On days when weather permits, we open our windows to allow the circulation of fresh air.
• We’ve installed touch-free hand sanitizer stations throughout our building.
• Employees who are not well are not working in the building.
COVID-19 POLICIES
WAIVER OF LIABILITY AGREEMENT:
Waiver/Release for Communicable Diseases Including COVID-19
ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT
In consideration of my attendance and/or participation in programs and related events and activities presented on behalf of The Carnegie Center for Literacy & Learning (hereinafter “The Carnegie Center”), my attendance and participation serve as my agreement, acknowledgement, and understanding that I do hereby communicate and acknowledge:
1. Participation includes possible exposure illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19 and any variants thereof.
2. By willingly and voluntarily attending in-person classes, activities or events at The Carnegie Center , I declare myself and my child (if applicable) is/are not experiencing or exhibiting any COVID-19 related symptoms as outlined by the Center for Disease Control and Prevention (CDC), Kentucky Department for Public Health, and the Lexington-Fayette County Health Department.
3. By willingly and voluntarily attending in-person classes, activities, or events, I acknowledge that myself and my child (if applicable) must follow the safety protocols that have been implemented by The Carnegie Center and posted in the building/property including practicing social distancing and maintaining separation of six feet from individuals and properly wearing masks/facial coverings as authorized and/or modified by The Carnegie Center Executive Director or Staff.
4. By willingly and voluntarily attending in-person classes, activities, or events, I declare that myself and my child (if applicable) have not traveled internationally or to a highly impacted COVID-19 area within the United States during the last (14) days.
5. By willingly and voluntarily attending in-person classes, activities, or events, I declare that myself and my child (if applicable) is/are not aware, nor do I believe I/we have been recently exposed to a person or animal with a positive and confirmed case of COVID-19.
6. I understand that for the purpose of this and all events, The Carnegie Center reserves the right to change and/or modify its masking and social distancing policy with respect to perceived or realized threats of viral infection to protect the health and safety of its Staff, Patrons, Sponsors, Partners, and Guests, and in doing so, individuals who do not adhere to the stated policy changes may be asked to leave the premises.
7. I acknowledge I may be removed from an event or asked to leave the premises/property by The Carnegie Center personnel should I exhibit any behavior in direct contrast or opposition to The Carnegie Center guidelines and safety measures.
8. I willingly and voluntarily agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of The Carnegie Center on-duty Staff immediately.
9. I understand that in the case of multiple/group ticket purchases, all attendees using the respective tickets are subject to the terms of this waiver in order to participate in any events, and I further understand and acknowledge that I have or will communicate the terms and requirements of this event waiver to my guest(s) for whom I have purchased tickets prior to their entering the facility to participate in events at The Carnegie Center.
10. If a class, activity, or event attendee is a minor child under the age of eighteen years, said attendee’s adult parent(s) or legal guardian(s) shall be jointly and severally liable for the attendee’s performance of this event attendee waiver and release of liability form and represent and warrant to The Carnegie Center that they have full authority to enter into this event attendee waiver and release of liability form on behalf of such minor.
11. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation, and DO HEREBY RELEASE AND HOLD HARMLESS The Carnegie Center, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property.
BY PURCHASING A TICKET AND/OR VOLUNTARILY PARTICIPATING IN EVENTS AT THE CARNEGIE CENTER FOR LITERACY & LEARNING, I FULLY ACKNOWLEDGE THAT I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND DO SO FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
MASKS ARE REQUIRED
Masks are optional for ALL classes, events, & tutoring sessions that take place at the Carnegie Center.
Did you arrive without a mask, but would feel more comfortable in a shared space with a mask? You can always go to the front desk and ask for a mask.
The Carnegie Center's mask policy is subject to change to combat a rise in the COVID-19 Community Levels.
If a mandatory mask policy will be unsustainable for an event, registrants and attendees will be notified in the event details and referred to our Waiver of Liability Agreement for Communicable Diseases.
For Non-Ticketed Events & "At The Door" Ticket Purchases – Attendees Must Register Upon Arrival:
STEP 1: Locate the Attendee Registration Table and Laptop.
STEP 2: Type your FULL NAME, PHONE NUMBER, & EMAIL ADDRESS into the Event Attendees Registration Spreadsheet.
If you need assistance completing STEP 2, please ask a Carnegie Center Staff Member or Event Personnel for help.
